RECOMMENDATION:

Adopt a resolution:

 

A.

Waiving the Request for Proposal process

 

B.

Authorizing the President of the Board of Supervisors to execute an agreement with Sequoia Health Services to provide tertiary care for the term January 1, 2006 to December 31, 2007, for a maximum amount payable of $500,000.

 

C.

Authorizing the Chief Executive Officer (CEO) of San Mateo Medical Center (SMMC) or the CEO's designee to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.

BACKGROUND:

San Mateo County is responsible for providing medical care to its medically indigent adult population. San Mateo Medical Center (SMMC) is a general acute care hospital and does not provide certain highly specialized services. A contract with Sequoia Health Services ensures that specialty services are readily available.

 

DISCUSSION:

Services provided under this agreement are for services that require technical capabilities that are not presently provided at San Mateo Medical Center. Most services are in the areas of radiation therapy, noninvasive procedures, cardiac specialty services, sleep lab and medical/surgical acute care.

In addition, Sequoia Hospital outpatient and lab rates are tied to the Medicare Fee Schedule. All patients referred to Sequoia for tertiary service undergo utilization review and minimize hospital stays and maintain control over costs.

San Mateo Medical Center is requesting a waiver of the RFP process because the facility seeks contracts with all area hospitals for tertiary care. SMMC has established contracts with most of the hospitals on the Peninsula to provide services that SMMC cannot. Contracting with multiple hospitals will ensure that there are a sufficient number of alternative options in a time when many hospitals have capacity issues. Seton has been a partner of SMMC for years and has provided high quality cost-effective care.

The contractor has assured compliance with the County's Contractor Employee Jury Service Ordinance, as well as all other contract provisions that are required by County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.

County Counsel and Risk Management have reviewed and approved this Agreement.

 

FISCAL IMPACT:

The term of the Agreement with Sequoia is January 1, 2006 to December 31, 2007 with a maximum amount payable not to exceed $500,000. Delay in processing this Agreement is due to negotiations. Funds are included in SMMC’s FY2005-06 Approved Budget and funds will be included in future year budget requests.
