AMENDMENT TO AGREEMENT
BETWEEN THE COUNTY OF SAN MATEO AND
Pacific Cardiovascular Surgeons, Inc.

THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and Pacific Cardiovascular Surgeons, Inc. , hereinafter called "Contractor";

W I T N E S S E T H:
|
|
|
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
|
|
|
WHEREAS, the parties entered into an Agreement for cardiology services to medically indigent adult (MIA) patients referred by San Mateo Medical Center (SMMC) to other facilities; and
|
|
|
WHEREAS, the parties wish to amend the Agreement to increase the contract by SIXTY FIVE THOUSAND DOLLARS ($65,000.00) to ONE HUNDRED AND FIFTEEN THOUSAND DOLLARS ($115,000.00).

NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:

Amount and Source of Payment of the agreement is amended to read as follows:
Contractor will be reimbursed 100% of the Medicare Part B Participating Fee Schedule for San Mateo County. Contractor shall bill and be paid per HCFA coding guidelines. Funds are included in the 2006-07 Adopted Budget and will be included in the 2007-08 Budget. Under no circumstances shall this agreement exceed ONE HUNDRED AND FIFTEEN THOUSAND DOLLARS ($115,000.00)

All other terms and conditions of the agreement dated January 27, 2006, between the County and Contractor shall remain in full force and effect.
