BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH HOWARD GUREVITZ, M.D. TO INCREASE THE MAXIMUM OBLIGATION BY $51,000 TO A NEW MAXIMUM OF $150,000 TO PROVIDE PSYCHIATRY SERVICES FOR THE TERM JULY 1, 2005 THROUGH JUNE 30, 2008; AND 2) AUTHORIZING THE DIRECTOR OF HEALTH OR THE DIRECTOR'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on June 29, 2005, the County Manager approved an Agreement with Howard Gurevitz, M.D. for the provision of psychiatry services for the term of July 1, 2005 through June 30, 2008 for a maximum obligation of $99,000; and

 

WHEREAS, both parties now wish to amend the Agreement with Howard Gurevitz, M.D., to increase the maximum obligation by $51,000 to a new maximum obligation of $150,000 with no change to the term; and

 

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment to the Agreement:

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of Health or the Director's designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
