RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE GRANT AGREEMENT WITH THE PENINSULA HEALTHCARE DISTRICT TO FUND THE CHILDREN’S HEALTH INITIATIVE BY INCREASING THE GRANT AMOUNT BY $682,250, AND EXTENDING THE TERM TO JANUARY 1, 2003 THROUGH DECEMBER 31, 2007.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Board of Supervisors, by Resolution Numbers 65792, 67295 and 67976 authorized an Agreement and Amendments with the Peninsula Healthcare District for a total amount of $2,114,500, semi-annually beginning January 2003 through July 2006 for funding for the Children’s Health Initiative; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and approval, an Amendment to the Agreement, whereby the Peninsula Healthcare District will continue to support the Children’s Health Initiative; and

WHEREAS, the parties wish to enter into an Amendment to the Agreement to provide funding for the Children’s Health Initiative increasing the amount by $682,250 to be funded on a semi-annual basis January 2007 and July 2007:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
