RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE THE AMENDMENT 2006-04 TO THE HOSPITAL AGREEMENT WITH THE HEALTH PLAN OF SAN MATEO TO EXTEND THE TERM OF THE AGREEMENT FROM DECEMBER 1, 1993 THROUGH DECEMBER 31, 2006 TO DECEMBER 1, 1993 THROUGH DECEMBER 31, 2007.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County of San Mateo and the Health Plan of San Mateo are parties to a Hospital Agreement; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a resolution authorizing the President of the Board of Supervisors to execute Amendment 2006-4 to the Hospital Agreement reference to which is hereby made for further particulars, whereby it extends the term of the Agreement with Health Plan of San Mateo by one year to December 31, 2007; and

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY RESOLVED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
