RESOLUTION 1) APPROVING THE COUNTY PLAN, BUDGET, AND PROGRAM ADMINISTRATIVE POLICIES FOR THE CALIFORNIA DEPARTMENT OF HEALTH SERVICES OUTREACH, ENROLLMENT, RETENTION, AND UTILIZATION; AND 2) ACCEPTING ALLOCATION FUNDING TO THE CHILDREN’S HEALTH INITIATIVE IN THE AMOUNT OF $845,343 FOR THE TERM FEBRUARY 2, 2007 THROUGH JUNE 30, 2009; AND 3) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT OR THE DIRECTOR'S DESIGNEE TO EXECUTE AMENDMENTS TO THE COUNTY PLAN, BUDGET, AND PROGRAM ADMINISTRATIVE POLICIES WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), OR MODIFY THE COUNTY PLAN, BUDGET, AND PROGRAM ADMINISTRATIVE POLICIES TERMS OR SERVICES IN A MANNER CONSISTENT WITH THE THEN EXISTING FINANCIAL OBLIGATION

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California; that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance of the County Plan, Budget and Program Administrative Policies, reference to which is hereby made for further particulars, whereby the California Department of Health Services will provide allocation funding for outreach enrollment activities to enroll and retain eligible children in Medi-Cal and Healthy Families; and,

WHEREAS, the term of the County Plan, Budget and Program Administrative Policies for this state allocation funding is February 2, 2007 through June 30, 2009, for a total obligation of $845,343; and,

WHEREAS, this Board of Supervisors has examined the County Plan, Budget and Program Administrative Policies ;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the County Plan, Budget and Program Administrative Policies are approved and the allocation funding is accepted.

BE IT FURTHER RESOLVED that the Health Department Director or the Director’s designee is authorized to execute amendments to the County Plan, Budget, and Program Administrative Policies which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), or modify the County Plan, Budget, and Program Administrative Policies terms or services in a manner consistent with the then existing financial obligation
