AMENDMENT TO AGREEMENT
BETWEEN THE COUNTY OF SAN MATEO AND
MILLS-PENINSULA HEALTH SERVICES
|

THIS AMENDMENT TO THE AGREEMENT, entered into this ___ day of ______, 2007, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and Mills-Peninsula Health Services, hereinafter called "Contractor";
|
|
|
W I T N E S S E T H:
|
|
|
WHEREAS, pursuant to Government Code, Section 31000, the County may contract with independent contractors for the furnishing of services such as those provided by the Contractor to or for County or any Department thereof;
|
|
|
WHEREAS, the parties entered into an Agreement for specialty Inpatient and “O/P (Outpatient) Services” on February 7, 2006; and
|
|
|
WHEREAS, the parties wish to amend the Agreement to reflect concluded negotiations to adjust the “O/P All Other Services” paid at 55 % of (billed) charges to 35% of (billed) charges, and
WHEREAS, the parties wish to amend the Agreement to reduce the contract by $60,000.

|
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:

All other terms and conditions of the agreement dated February 7, 2006, between the County and the Contractor shall remain in full force and effect.
|
|
|
|
|
|
