BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH ONE EAST PALO ALTO FOR CONSULTATION AND OUTREACH AND ENGAGEMENT SERVICES, INCREASING THE MAXIMUM BY $130,583 TO A NEW MAXIMUM OF $155,183 AND EXTENDING THE TERM THROUGH JUNE 30, 2008; AND 2) AUTHORIZING THE DIRECTOR OF HEALTH OR THE DIRECTOR'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on October 2, 2006, the Director of Health approved an Agreement with One East Palo Alto for the provision of facilitation and coordination services for the East Palo Alto Mental Health Community Advisory Group for the term September 1, 2006 through June 30, 2007, for a maximum of $24,600; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, whereby One East Palo Alto shall provide mental health outreach and engagement services in the East Palo Alto region, reference to which is hereby made for further particulars; and

 

WHEREAS, the Amendment to the Agreement shall increase the maximum obligation by $130,583 from $24,600 to $155,183 and extend the term through June 30, 2008; and

 

WHEREAS, the Board of Supervisors has examined and approved the Amendment to the Agreement as to form and content and desires to enter into same:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of Health or the Director's designee to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
