BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

ADOPT A RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH CALIFORNIA ADVANCED IMAGING MEDICAL ASSOCIATES, INC. TO PROVIDE PROFESSIONAL RAIDOLOGY AND OTHER DIAGNOSTIC IMAGING SERVICES FROM APRIL 1, 2007 THROUGH DECEMBER 31, 2007, IN AN AMOUNT NOT TO EXCEED $1,414,645. AUTHORIZING THE CHIEF EXECUTIVE OFFICER (CEO) OR THE CEO'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS San Mateo Medical Center (SMMC) requires professional services in the areas of radiology and other diagnostic imaging services, and

 

WHEREAS, the County has determined that California Advanced Imaging Medical Associates, Inc. (CAIMA) possesses the skills and abilities necessary to provide radiology and other diagnostic imaging services to SMMC; and

 

    WHEREAS there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby CAIMA shall provide radiology and other diagnostic imaging services to SMMC, and for SMMC, at other SMMC sites for the period of April 1, 2007 through December 31, 2007, for a total amount not to exceed $1,414,645; and

 
 
 

    WHEREAS, this Board considered the Agreement and has approved it as to both form and content and desires to enter into it.

    NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.

BE IT FURTHER RESOLVED that the Chief Executive Officer (CEO) of San Mateo Medical Center, or the CEO's designee, shall be authorized to execute amendments to the Agreement which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
