California Department of Aging (CDA) Agreement Number MS-0708-13

 

RECOMMENDATION:

Adopt a Resolution:

   

1.

Authorizing the President of the Board to execute Agreement Number MS-0708-13 with the California Department of Aging, in the amount of $857,000 for the continuation of Multipurpose Senior Services Program (MSSP) services for the term of July 1, 2007 to June 30, 2008; and

   

2.

Authorizing the Director of Health or the Director’s designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.

This Agreement contributes to this commitment and goal by providing clients with adult day care, attendant care, home health coordination, emergency response systems, home modifications, adaptive equipment, and transportation services, as well as comprehensive case management services. It is anticipated that a total of 250 clients will be served under this Agreement. It is projected that 22 clients (nine percent) may need to be placed in a Skilled Nursing Facility (SNF) due to their physical deterioration.

BACKGROUND:

Aging and Adult Services (AAS) provides adult protective services, case management, representative payee, conservatorship, and In-Home Supportive Services to functionally impaired adults and seniors. MSSP, one of the Division’s case management programs, provides comprehensive social and health care management services to Medi-Cal eligible at-risk elders through an agreement with the State. The program operates under a Medi-Cal waiver obtained by the State, which specifically allows Medi-Cal reimbursement for health care services provided in the home rather than in a more costly institutional setting.

 

The primary objectives of MSSP are to avoid the inappropriate placement of fragile older persons in nursing facilities and to foster independent living in their own communities. MSSP provides a care management team of social workers and public health nurses to assess client eligibility and need and to arrange and monitor the delivery of services. A client becomes ineligible for MSSP when the program’s services are no longer sufficient to enable the individual to remain safely at home. Though many clients stop participation because of necessary institutionalization, many others are able to avoid institutionalization altogether through MSSP’s services and are able to remain at home as clients with the program for the remainder of their lives.

 

DISCUSSION:

The Agreement includes funds for social workers and nurses to provide assessments and case management services. Funds are also made available through the Agreement to provide home modifications, purchase durable medical equipment and other supplies, and provide attendant care. In FY 2006-07 the State provided an augmentation of $117,615 for MSSP services. This augmentation is continued in this Agreement for FY 2007-08. These additional funds will be used to offset increases in salaries and benefits and to purchase additional services for clients.

 

County Counsel has reviewed and approved the Agreement and Resolution as to form.

 

FISCAL IMPACT:

The term of the Agreement is July 1, 2007 through June 30, 2008. The total amount of funding from the State through this Agreement is $857,000. All program costs and revenues for MSSP services are included in the Aging and Adult Service’s FY 2007-08 Recommended Budget. There is no Net County Cost associated with the submittal of this Agreement.
