Agreement with Sequoia Anesthesia Consultants

 

RECOMMENDATION:

Adopt a Resolution

    1. Authorizing the President of the Board to execute an Agreement with Sequoia Anesthesia Consultants to provide professional anesthesiology and pain management services from May 1, 2007 though April 30, 2009, in an amount not to exceed $3,134,000.

    2. Authorizing the San Mateo Medical Center (SMMC) Chief Executive Officer (CEO) or the CEO's designee to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.

BACKGROUND:

 

San Mateo County is responsible for providing access to professional medical care to its medically indigent residents. San Mateo Medical Center (SMMC), as a general acute care hospital, does not provide certain highly specialized services. A contract with Sequoia Anesthesia Consultants ensures that certain necessary specialty services are readily available.

Services provided under this Agreement are required medical services that are not presently provided by SMMC. The Agreement provides for the performance of anesthesiology and pain management services at SMMC Surgical Department on an on call basis, as needed and requested by the County.

SMMC entered into contract renewal discussions with Sequoia Anesthesia Consultants in February of 2007. The Agreement was delayed due to difficult rate negotiations and the transition of SMMC staff.

 

DISCUSSION:

Sequoia Anesthesia Consultants provides anesthesiology to SMMC patients during surgical procedures, invasive non surgical procedures and chronic pain management. Sequoia Anesthesia Consultants adheres to best practices in anesthesiology.

All patients referred to Sequoia Anesthesia Consultants undergo utilization review to minimize hospital stays and maintain control over costs.

The contractor has assured compliance with the County's Contractor Employee Jury Service Ordinance, as well as all other contract provisions that are required by County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.

County Counsel has reviewed and approved this Amendment to the Agreement. Risk Management has approved the insurance coverage.

FISCAL IMPACT:

Funds are included in the San Mateo Medical Center’s FY2006-07 Adopted Budget and in the FY2007-08 Recommended Budget and will be included in the FY 2008-09 Recommended Budget.

The previous Agreement was for a period of fifteen months, (15), and amounted to $2,490,000. The current contract is for a period of twenty four months, (24), and amounts to $3,134,000. The contract represents no rate increase and no reserve for exceeding the contract baseline; since that was not achieved in the prior contract.

In no event shall the County’s total fiscal obligation under this Amendment exceed THREE MILLION ONE HUNDRED AND THIRTY FOUR THOUSAND DOLLARS, ($3,134,000.)
