BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AGREEMENT NUMBER MS-0708-13 WITH THE CALIFORNIA DEPARTMENT OF AGING, IN THE AMOUNT OF $857,000 FOR THE CONTINUATION OF MULTIPURPOSE SENIOR SERVICES PROGRAM (MSSP) SERVICES FOR THE TERM OF JULY 1, 2007 TO JUNE 30, 2008; AND 2) AUTHORIZING THE DIRECTOR OF HEALTH OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby the Aging and Adult Services Division shall provide a Multipurpose Senior Services Program (MSSP) that provides comprehensive social and health support necessary to enable frail elderly to remain in their own homes; and

 

WHEREAS, the parties wish to enter into an Agreement to provide MSSP services for the period of July 1, 2007 through June 30, 2008, for a total obligation of $857,000; and

 

WHEREAS, this Board has examined the Agreement and has approved it as to both form and content:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Agreement Number MS-0708-13 for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of the Health Department or the Director’s designee be and is hereby authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
