AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND STAT NURSES INTERNATIONAL, INC.
THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and STAT NURSES INTERNATIONAL, INC. hereinafter called "Contractor";
W I T N E S S E T H:
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
WHEREAS, on June 27, 2006, pursuant to Board Resolution No. 068117, the parties entered into an Agreement for providing nursing and/or rehabilitation staff upon direct request from San Mateo Medical Center, Burlingame Long Term Care and the AIDS Program services; and
WHEREAS, the parties wish to amend the Agreement to authorize the assignment of it from Stat Nurses International, Inc. to Health Talent, Inc.].
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
1.	All references to “Stat Nurses International, Inc.” are amended to read “Health Talent, Inc.”
2	All other terms and conditions of the agreement dated June 27, 2006, between the County and Contractor shall remain in full force and effect.
