San Mateo Medical Center (SMMC) is annually awarded the Health Care for the Homeless (HCH) grant from the federal Bureau of Primary Health Care of the Health and Resource Services Administration to provide primary care, dental care, outreach, health education, case management, and supportive services to the San Mateo County’s homeless population.
HCH is designed to assist homeless adults, families and youth with access to and use of the SMMC’s continuum of primary care, dental care, mental health, substance abuse, and other support services. In Winter 2003, the HCH Advisory Committee developed a strategic plan for the HCH Program. The goals of the Strategic Plan include to: 1) Serve a large portion of the homeless population who are on the street or in shelters; 2) Provide one or more of the following services to homeless individuals: primary care services, dental care services, substance abuse and mental health treatment; 3) Eliminate barriers and provide better access to services by outreach to homeless individuals.
Following a Request for Proposal process, Ravenswood Family Health Center was one of six contractors awarded an agreement based on the priorities of the strategic plan. The term of the Agreement with Ravenswood Family Health Center was November 1, 2005 to October 31, 2006, with a maximum amount payable under the Agreement of $108,000.
On February 13, 2007, the Board approved an amendment to the Agreement increasing the maximum amount payable by $54,000 to a total of $162,000 and extending the term of the Agreement by six months to April 30, 2007 because the Health Care for the Homeless (HCH) Program was in the process of designing a Request for Proposal (RFP) that was to be issued in January 2007.

The HCH Program is extending this contract for an additional three months because it is in the process of designing a Request for Proposal (RFP). The HCH Program has created a Business Plan that has allowed it to analyze how it currently provides services to the homeless population in San Mateo County. As a result of the Business Plan, the Program is working on restructuring its service delivery system in order to continue to improve the quality of services and access to them. Once the service system is restructured, a RFP will be issued so organizations that serve individuals who are homeless can submit proposals and apply for funding to provide medical, dental, mental health, substance abuse and/or other case management services. This RFP Process will be completed by July 2007 and contracts will be awarded for the period beginning August 1, 2007.
The contractor has assured compliance with the County's Contractor Employee Jury Service Ordinance, as well as all other contract provisions that are required by County ordinance and administrative memoranda, including but not limited to insurance, hold harmless, non-discrimination and equal benefits.
County Counsel and Risk Management have reviewed and approved this Agreement.

This Amendment extends term of this Agreement by three months through July 31, 2007. The maximum amount of this Agreement is increased by $27,000, from $162,000 to $189,000. All funds for this Agreement are provided by the HCH grant. Expenditures and grant revenue for this Agreement are included in San Mateo Medical Center’s FY2006-07 Adopted Budget and FY2007-08 Recommended Budget. There is no Net County Cost.
