RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH TELECARE CORPORATION FOR SERVICES AT CORDILLERAS MENTAL HEALTH CENTER TO INCREASE THE MAXIMUM OBLIGATION BY $130,901 TO A NEW MAXIMUM OF $13,709,686 WITH NO CHANGE TO THE TERM OF THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on December 13, 2005, this Board of Supervisors approved an agreement with Telecare by Resolution 67720 for the provision of inpatient and residential psychiatric treatment services and direct observation therapy for non-adherent Tuberculosis patients at Cordilleras Mental Health Center for the period July 1, 2005 through June 30, 2007 for a maximum obligation of $13,575,865; and

 

WHEREAS, on June 21, 2006, the Director of Health approved a First Amendment to the Agreement, revising the payment structure for directly observed therapy services for non-adherent Tuberculosis patients and increasing the Agreement maximum by $2,920 to a new maximum of $13,578,785 with no change to the Agreement term; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Second Amendment to the Agreement with Telecare, reference to which is hereby made for further particulars, whereby the maximum obligation shall be increased by $130,901 to a new maximum of $13,709,686 with no change to the Agreement term; and

 

WHEREAS, the Board of Supervisors has examined and approved the Second Amendment to the Agreement as to form and content and desires to enter into same:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
