RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH ONSITE DENTAL CARE FOUNDATION TO PROVIDE DENTAL SERVICES TO SAN MATEO COUNTY’S HOMELESS POPULATION TO INCREASE THE MAXIMUM AMOUNT PAYABLE BY $85,808 TO $397,418, AND EXTENDING THE TERM OF THE AGREEMENT BY THREE MONTHS TO JULY 31, 2007

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, in 2003 San Mateo Medical Center was awarded a Health Care for the Homeless (HCH) grant from the federal Bureau of Primary Health Care for the Health and Resource Services Administration to provide primary care, dental care, outreach, health education, case management, and supportive services to San Mateo County’s homeless population; and

 

WHEREAS, following a Request for Proposal (RFP) process, Onsite Dental Care Foundation was awarded a contract based on the priorities of the HCH Program strategic plan; and

 

WHEREAS, the original term of the Agreement with Onsite Dental Care Foundation was from November 1, 2005 to October 31, 2006, with a maximum amount payable thereunder of $198,575; and

 

WHEREAS, on January 9, 2007, pursuant to Board Resolution No. 068476, the Agreement with Onsite Dental Care Foundation was amended to increase the maximum amount payable thereunder by $133,035 to $311,610 and extend the term there of for l six months to April 30, 2007, to allow for the designing of an RFP that was to be issued in January 2007; and

 

WHEREAS, this Board has determined that it is in the County’s best interest to further amend the Agreement to provide dental services by revising in their entirety Exhibits A and B, increasing the maximum amount by $85,808 to $397,418, and extending the term of the Agreement by three months to July 31, 2007, because of a delay in the RFP process; and

 

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
