RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH RAVENSWOOD FAMILY HEALTH CENTER TO PROVIDE PRIMARY CARE SERVICES TO SAN MATEO COUNTY’S HOMELESS POPULATION AMENDING IN THEIR ENTIRETY EXHIBITS A AND B, INCREASING THE MAXIMUM AMOUNT PAYABLE BY $27,000 TO A TOTAL OF $189,000 AND EXTENDING THE TERM OF THE AGREEMENT BY THREE MONTHS TO JULY 31, 2007

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, in 2003 San Mateo Medical Center was awarded the Health Care for the Homeless (HCH) grant from the federal Bureau of Primary Health Care of the Health and Resource Services Administration to provide primary care, dental care, outreach, health education, case management, and supportive services to the San Mateo County’s homeless population;

WHEREAS, following a Request for Proposal (RFP) process, Ravenswood Family Health Center was awarded an agreement under the HCH grant, the original term of which was November 1, 2005 to October 31, 2006, with a maximum amount payable thereunder of $108,000; and

WHEREAS, the original Agreement was amended on February 13, 2007 to extend the Agreement with Ravenswood by six months to April 30, 2007, and increase the maximum amount payable by $54,000 to $162,000; and

WHEREAS, the County desires to further amend the Agreement to extend its term for an additional three months, to increase the maximum amount payable thereunder by $27,000 to $189,000 and to make other changes thereto.

WHEREAS, this Board has been presented with a form of the Amendment to the Agreement, has examined and approved it as to both form and content, and desires to enter into it.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
