RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE THE AGREEMENTS WITH ADDUS HEALTHCARE, INC.; AMERICAN CAREQUEST, INC.; MEDICAL CARE PROFESSIONALS; AND NP PLUS, LLC., FOR HOME HEALTH AND ATTENDANT CARE SERVICES FOR THE TERM JULY 1, 2007 THROUGH JUNE 30, 2010 FOR THE COLLECTIVE AMOUNT OF $970,000 FOR FY 2007-08; AND 2) AUTHORIZING THE DIRECTOR OF HEALTH OR THE DIRECTOR'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) FOR EACH AGREEMENT AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Agreements, references to which are hereby made for further particulars, whereby Addus HealthCare, Inc., American CareQuest, Inc., Medical Care Professionals, and NP Plus, LLC., shall provide Home Health and Attendant Care services for the Multipurpose Senior Services Program, AIDS Case Management and Waiver Programs, Linkages/Respite, Centralized Intake/Adult Protective Services, and the Public Guardian program; and

WHEREAS, all parties now wish to enter into Agreements to provide Home Health and Attendant Care services for the period of July 1, 2007 to June 30, 2008, for a total obligation of $970,000 collectively; and

WHEREAS, the amounts to be provided for FY 2008-09 and FY 2009-10 have yet to be determined and amendments will be brought before the Board of Supervisors for approval to include the additional amounts when that information is available; and

WHEREAS, this Board has examined the Agreements and has approved them as to both form and content:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Agreements with Addus HealthCare, Inc., American CareQuest, Inc., Medical Care Professionals, and NP Plus, LLC., for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of Health or the Director's designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate) for each Agreement and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
