RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH WORKFORCE MEDICAL GROUP TO PROVIDE PRE-PLACEMENT AND OCCUPATIONAL HEALTH MONITORING PHYSICAL EXAMINATIONS AND SERVICES TO COMPLY WITH FEDERAL AND STATE MANDATES, EXTENDING THE TERM THROUGH DECEMBER 31, 2007, INCREASING THE AMOUNT BY $200,000 TO $762,500, AND ADDING COMPLIANCE WITH CONTRACTOR EMPLOYEE JURY SERVICE ORDINANCE LANGUAGE TO THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the County must provide pre-placement and occupational health services to its employees to comply with Federal and State mandates;

WHEREAS, the County previously entered into an three-year agreement with Workforce Medical Group in the amount of $562,500 to provide occupational health services;

WHEREAS, the County desires to extend the contract term for three months through December 31, 2007 and increase the contract amount by $200,000 for a total Not to Exceed Amount of $762,500;

WHEREAS, there has been presented to this Board of Supervisors a proposed amendment to the agreement for its consideration and acceptance reference to which is hereby made for further particulars; and;

WHEREAS, this Board of Supervisors has examined and approved the amendment to the agreement as to form and content and desires to enter into same;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to, execute said amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
