BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

ADOPT A RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH RONALD GREENWALD, M.D. FOR $618,000 TO PROVIDE NEUROSURGERY SERVICES FOR THE PERIOD JULY 1, 2007 THROUGH JUNE 30, 2009 AND THE SAN MATEO MEDICAL CENTER (SMMC) CHIEF EXECUTIVE OFFICER (CEO) OR THE CEO'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Ronald Greenwald M.D. shall provide professional neurosurgery services starting July 1, 2007; and

WHEREAS, this Board considered the form of the Agreement and has approved it as to both form and content and desires to enter into it.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED THAT: the Chief Executive Officer (CEO) of San Mateo Medical Center, or the CEO's designee, shall be authorized to execute amendments to the Agreement which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
