RESOLUTION: 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH CREEKSIDE MENTAL HEALTH REHABILITATION PROGRAM FOR THE PROVISION OF MENTAL HEALTH REHABILITATION SERVICES AND LONG TERM CARE FOR SERIOUSLY MENTALLY ILL SAN MATEO COUNTY RESIDENTS, EXTENDING THE TERM THROUGH JUNE 30, 2009 AND INCREASING THE MAXIMUM OBLIGATION BY $298,505 TO A NEW MAXIMUM OF $398,505; AND 2) AUTHORIZING THE DIRECTOR OF HEALTH OR THE DIRECTOR'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE) AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on February 2, 2007, the County Manager approved an Agreement with Creekside Mental Health Rehabilitation Program by Agreement Number 61000-07-C172 for the provision of long term care and mental health rehabilitation services for the term September 1, 2006 through June 30, 2007, for a maximum obligation of $100,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby the maximum obligation is being increased by $298,505 to a new maximum obligation of $398,505 and the term of the Agreement is being extended through June 30, 2009; and

WHEREAS, the Board of Supervisors has examined and approved the Amendment to the Agreement as to form and content and desires to enter into same:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of Health or the Director's designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate) and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
