RESOLUTION 1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE GRANT AGREEMENT WITH SILICON VALLEY COMMUNITY FOUNDATION, EXTENDING THE GRANT TERM FROM DECEMBER 1, 2006 THROUGH NOVEMBER 30, 2007 TO DECEMBER 1, 2006 THROUGH MAY 31, 2008 WITH NO CHANGE TO THE GRANT AGREEMENT AMOUNT OF $100,000; AND 2) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT OR THE DIRECTOR'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on October 16, 2006, the Board approved a Grant Agreement with Silicon Valley Community Foundation, previously Peninsula Community Foundation, through Resolution Number 068334 supporting implementation of the Healthier Outcomes Through Multi-disciplinary Engagement (HOME) Team; and

 

WHEREAS, the parties wish to amend this Grant Agreement extending the Grant Term from December 1, 2006 through November 30, 2007 to December 1, 2006 through May 31, 2008, with no change to the Grant Agreement amount of $100,000; and

 

WHEREAS, all other terms and conditions of the Grant Agreement signed October 17, 2006, between the County and Contractor shall remain in full force and effect:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is authorized to execute said Amendment to the Grant Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest to the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of the Health Department or the Director's designee is authorized to execute contract amendments which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
