RESOLUTION 1. AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH BAY AREA UROLOGY MEDICAL GROUP, INC. TO PROVIDE UROLOGY SERVICES TO EXTEND THE TERM FROM MARCH 1, 2006 THROUGH AUGUST 31, 2007 TO MARCH 1, 2006 THROUGH DECEMBER 31, 2007 AND TO INCREASE THE AGREEMENT MAXIMUM FROM $474,398.68 TO AN AMOUNT NOT TO EXCEED $557,626.52 2. AUTHORIZING THE SAN MATEO MEDICAL CENTER (SMMC) CHIEF EXECUTIVE OFFICER (CEO) OR THE CEO'S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY'S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

 

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, San Mateo Medical Center (SMMC) has a need for professional urology medical services, and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement to extend the contract for four months from September 1, 2007 through December 31, 2007, reference to which is hereby made for further particulars, whereby Bay Area Urology Medical Group, Inc., shall provide such urology services; and;

WHEREAS, this Board has approved the Amendment to the Agreement as to both form and content and desires to enter into it:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo and the Clerk of this Board shall attest the President's signature thereto.

BE IT FURTHER RESOLVED the Chief Executive Officer (CEO) of SMMC, or the CEO's designee, shall be authorized to execute amendments to the Agreement which modify the County's maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
