RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH ANTHONY THEKKEK, DBA BURLINGAME HACIENDA TO PROVIDE BOARD AND CARE SERVICES, INCREASING THE AGREEMENT MAXIMUM BY $38,175 TO A NEW MAXIMUM OF $190,065, WITH NO CHANGE TO THE TERM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on December 14, 2004, the Board of Supervisors approved an Agreement with Anthony Thekkek, dba Burlingame Hacienda, by Resolution Number 067091 for the provision of board and care services to clients of San Mateo County for the term July 1, 2004 through June 30, 2007, for a maximum obligation of $151,890; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby the maximum obligation is being increased by $38,175 to a new maximum obligation of $190,065; and

WHEREAS, the Board of Supervisors has examined and approved the Amendment to the Agreement as to form and content and desires to enter into same:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
