RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH CALIFORNIA EMERGENCY PHYSICIANS TO: 1. ADD GO LIVE EMERGENCY DEPARTMENT (ED) TRAININGS IN ORDER TO DOCUMENT ED SERVICES ELECTRONICALLY 2. REIMBURSE GO LIVE TRAININGS AT NO MORE THAN AGREEMENT PHYSICIAN AND PHYSICIAN ASSISTANT REIMBURSEMENT RATES AND REIMBURSE ADMINISTRATIVE FEES AT NO MORE THAN EIGHT PERCENT (8%) 3. TOTAL REIMBURSEMENT FOR GO LIVE ACTIVITIES SHALL NOT EXCEED THIRTY TWO THOUSAND AND FIFTEEN DOLLARS ($32,015)

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on April 4, 2006 this Board of Supervisors, by Resolution 67907, authorized an Agreement with California Emergency Physicians (CEP) in the amount of $2,691,282.20 for the term January 1, 2006 through December 31, 2006 for the purpose of providing emergency medical services to the San Mateo Medical Center (SMMC); and

WHEREAS, SMMC had a need to train CEP staff to convert ED service information to electronic record keeping, and

WHEREAS, the parties hereto now wish to amend the Agreement to include services rendered in training for the conversion to electronic record keeping, and

WHEREAS, the parties hereto wish to amend the Agreement to reimburse CEP

for the training activities related to electronic record keeping, and

WHEREAS, there has been presented to this Board for its consideration and acceptance an Amendment to the Agreement, reference to which is hereby made, whereby CEP shall be reimbursed for training activities performed; and

WHEREAS, this Board has considered the Amendment to the Agreement, has examined and approved it as to both form and content, and desires to enter into it.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signatures thereto.
