RESOLUTION AUTHORIZING EXECUTION OF AMENDMENT 01 TO THE AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF HEALTH SERVICES, STD CONTROL BRANCH FOR STD CONTROL ACTIVITIES, WHICH COMBINES THE CHLAMYDIA SCREENING PROJECT (CLASP) INTO THE STD COMMUNITY INTERVENTIONS PROGRAM (SCIP) FOR THE TERM OF JULY 1, 2004 THROUGH JUNE 30, 2009

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, in February 2005 the County Manager executed an Agreement with the California Department of Health Services STD Control Branch for a grant in the amount of $80,000 for the term of July 1, 2004 through June 30, 2009, to provide chlamydia and other STD awareness prevention activities with special emphasis on at-risk youth populations, under a State/local collaboration called the STD Community Interventions Program, or SCIP; and

WHEREAS, the State wishes to expand the scope of SCIP activities for FY 2007-09 and incorporate funding for the Chlamydia Screening Project (ClaSP) for FY 2007-09 into the existing SCIP agreement; and

WHEREAS, Amendment 01 expands the scope of work of SCIP activities, and combines funding for ClaSP activities in the amount of $80,000 for FY 2007-09 for a total grant award of $160,000; and

WHEREAS, this Board has been presented with this Amendment 01 to the Agreement and has examined and approved it as to both form and content and desires to enter into same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Amendment 01 to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
