RESOLUTION APPROVING A THIRD AMENDMENT

TO THE AGREEMENT WITH ANGELICA TEXTILE SERVICES, INC.

TO EXTEND THE TERM OF THE AGREEMENT TO PROVIDE LINEN AND LAUNDRY MANAGEMENT AT SAN MATEO MEDICAL CENTER (SMMC) AND BURLINGAME LONG TERM CARE BY FOUR MONTHS TO DECEMBER 31, 2007 AND INCREASE THE AMOUNT BY $190,104 TO $3,214,130

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on June 29, 2004, the Board of Supervisors, by Resolution Number 66776, and subsequent amending Resolution Nos. 067173 and 068863, authorized an Agreement with Angelica Textile Services, Inc. to provide linen and laundry management at San Mateo Medical Center (SMMC) and Burlingame Long Term Care; and

WHEREAS, a Request for Proposal (RFP) for linen services has been transitioned to a Bid List and is currently with the Purchasing Department in Redwood City and has not yet been finalized; and

WHEREAS, SMMC will have a continuing need for linen service while the Bid process is pending; and

WHEREAS, this Board has been presented with a form of Amendment to the Agreement to extend its term by four months, and has examined and approved it as to both form and content and desires to enter into the same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute the Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
