AMENDMENT TO AGREEMENT
BETWEEN THE COUNTY OF SAN MATEO AND
BAY AREA UROLOGY MEDICAL GROUP

THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and BAY AREA UROLOGY MEDICAL GROUP, hereinafter called "Contractor";

|
W I T N E S S E T H:
|
|
|
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
|
|
|
WHEREAS, the parties entered into an Agreement for Urology services at San Mateo Medical Center on May 2, 2006; and
|
|
|
WHEREAS, the parties wish to amend the Agreement to increase the amount of the Agreement by $22,194.08 to $579,820.64.
|
|
|
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
|
|
|
|

Exhibit “B” of the agreement is amended to read as follows:
The total amount of this Agreement shall not exceed FIVE HUNDRED SEVENTY NINE THOUSAND, EIGHT HUNDRED TWENTY DOLLARS AND SIXTY FOUR CENTS, ($579,820.64)

All other terms and conditions of the agreement dated May 2, 2006, between the County and Contractor shall remain in full force and effect.
