AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND PACIFIC FOOTCARE PODIATRY GROUP, INC.
THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and PACIFIC FOOTCARE PODIATRY GROUP, INC., hereinafter called "Contractor";
W I T N E S S E T H:
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
WHEREAS, the parties entered into an Agreement for podiatry services on February 7, 2006 for a term of October 1, 2005 to September 30, 2007; and
WHEREAS, the parties wish to amend the Agreement to extend the term of the Agreement by four months to a new term of October 1, 2005 to January 31, 2008 and increase the amount by $29,944.08 from $179,664.50 to $209,608.58.
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
1.	Section 3 Term and Termination of the agreement is amended to read as follows: 3.1 Term This agreement shall commence on October 1, 2005, and shall continue for two years and four months. Unless sooner terminated, this Agreement shall expire and be of no further force and effect as of the end of business on the 31st day of January 2008.
2.	Paragraph 5 in B, Outpatient Podiatry of SCHEDULE B of the original Agreement is amended to read as follows:
	5,	Contractor’s quarterly compensation will be no less than a minimum amount specified below:
		Year I (Monthly = $4,502.24) October 1, 1995 through December 31, 2005 January 1, 2006 through march 31, 2006 April 1, 2006 through June 30, 2006 July 1, 2006 through September 30, 2006 Year 2 (Monthly = $4,502.24) October 1, 2006 through December 31, 2006 January 1, 2007 through March 30, 2007 April 1, 2007 through June 30, 2007 July 1, 2007 through September 30, 2007 October 1, 2007 through January 31, 2008 (4 months) (Monthly = $7486.02)	$13,506.72 $13,506.72 $13,506.72 $13,506.72 $13,506.72 $13,506.72 $13,506.72 $13,506.72 $29,944.08
3.	Last Paragraph in SCHEDULE B of the original Agreement is amended to read as follows: Total payment for services under this Agreement will not exceed TWO HUNDRED NINE THOUSAND SIX HUNDRED EIGHT DOLLARS AND FIFTY- EIGHT CENTS ($209,608.58).
4.	All other terms and conditions of the agreement dated February 7, 2006, between the County and Contractor shall remain in full force and effect.
