RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH PACIFIC HEALTH CONSULTING GROUP, EXTENDING THE CONTRACT END DATE FROM DECEMBER 31, 2007 TO JUNE 30, 2008

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Blue Ribbon Task Force on Adult Health Care Coverage Expansion (Task Force) process, facilitated by Pacific Health Consulting Group, has brought together representatives from private and public health sectors, labor, and business to provide recommendations on options for providing comprehensive healthcare access and/or insurance coverage targeted at uninsured adults in San Mateo County living at or below 400% Federal Poverty Level; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, a Second Amendment to the Agreement with Pacific Health Consulting Group to further the work of the Task Force; and

WHEREAS, the proposed Amendment extends the end date of the Agreement with Pacific Health Consulting Group from December 31, 2007 to June 30, 2008, with no change to the maximum obligation of $122,030; and

WHEREAS, this Board has been presented with the Amendment and has examined and approved it as to both form and content and desires to enter into the same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is authorized to execute said Amendment to the Agreement with Pacific Health Consulting Group for and on the behalf of the County of San Mateo, and the Clerk of this Board shall attest to the President’s signature thereto.
