RESOLUTION AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE AN ACCEPTANCE OF ALLOCATION AGREEMENT FROM THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH, TOBACCO CONTROL SECTION, FOR THE TOBACCO PREVENTION PROGRAM IN THE AMOUNT OF $150,000 FOR THE TERM OF JULY 1, 2007 THROUGH JUNE 30, 2008, AND TO EXECUTE ALL SUBSEQUENT ACCEPTANCE OF ALLOCATION AGREEMENTS THROUGH JUNE 30, 2010.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration an Acceptance of Allocation Agreement, reference to which is hereby made for further particulars, whereby the State of California Department of Public Health (CDPH) will provide funding for tobacco prevention programs in San Mateo County in the amount of $150,000 for the term of July 1, 2007 through June 30, 2008, and;

 

WHEREAS, the State has requested that every effort be made to streamline the execution of the Tobacco Prevention Program Agreements and renewal contracts, and;

 

WHEREAS, the Director of the Health Department has been listed as a Designated Signatory for the San Mateo County Tobacco Prevention Program on the CDPH Tobacco Control Section’s Online Tobacco Information System; and

 

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement;

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Director of the Health Department is authorized to execute said CDPH Acceptance of Allocation Agreement for and on behalf of the County of San Mateo; and

 

BE IT FURTHER RESOLVED that the Director of the Health Department or the Director's designee is authorized to execute any and all future Acceptance of Allocation Agreements through June 30, 2010.
