BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

1) RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH WORKFORCE MEDICAL GROUP IN AN AMOUNT NOT TO EXCEED $827,736 TO PROVIDE PRE-PLACEMENT AND OCCUPATIONAL HEALTH MONITORING PHYSICAL EXAMINATIONS AND SERVICES TO COMPLY WITH FEDERAL AND STATE MANDATES FOR THE TERM JANUARY 1, 2008 THROUGH DECEMBER 31, 2010.

2) AUTHORIZING THE DIRECTOR OF HUMAN RESOURCES OR THE DIRECTOR’S DESIGNEE TO EXECUTE COTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED ERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the County must provide pre-placement and occupational health services to its employees to comply with Federal and State mandates;

 

WHEREAS, on August 31, 2004, the Board of Supervisors, by Resolution Number 066872 and subsequent amending Resolution Number 068923, authorized an Agreement with Workforce Medical Group, Inc. to provide occupational health services;

 

WHEREAS, the Human Resources Department, after conducting a Request for Proposal, wishes to enter into an Agreement with Workforce Medical Group, Inc. effective January 1, 2008 through December 31, 2010 in the amount not to exceed $827,736;

 

WHEREAS, there has been presented to this Board of Supervisors a proposed agreement for its consideration and acceptance reference to which is hereby made for further particulars; and;

 

WHEREAS, this Board of Supervisors has examined and approved the amendment to the agreement as to form and content and desires to enter into same;

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to, execute said amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and the Director of Human Resources or the Director’s designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
