RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH PACIFIC FOOTCARE PODIATRY GROUP, INC. TO PROVIDE PODIATRY SERVICES EXTENDING THE TERM BY FOUR MONTHS FOR A NEW PERIOD OF OCTOBER 1, 2007 THROUGH JANUARY 31, 2008 AND INCREASING THE AMOUNT BY $29,944.08 TO A MAXIMUM PAYABLE OF $209,608.58

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on February 7, 2006, the Board of Supervisors, by Resolution 67815, authorized an Agreement with Pacific Footcare Podiatry Group, Inc., to provide podiatry services to the San Mateo Medical Center for the term October 1, 2005 to September 30, 2007; and

 

WHEREAS, the San Mateo Medical Center has a continuous need for podiatry services and the Pacific Footcare Podiatry Group, Inc. would expand provider coverage of uninterrupted podiatry clinic and surgical services; and

 

WHEREAS, the parties now wish to amend the Agreement by extending the term of the Agreement by four months to a new term of October 1, 2005 to January 31, 2008, and increasing the maximum amount by $29,944.08, from $179,664.50 to $209,608.58; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made, whereby Pacific Footcare Podiatry Group, Inc. shall provide professional podiatry services; and

 

WHEREAS, this Board has been presented with a form of the Amendment and has examined and approved it as to both form and content and desires to enter into it.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
