RESOLUTION 1) AUTHORIZING THE ACCEPTANCE OF A GRANT FROM SILICON VALLEY COMMUNITY FOUNDATION IN THE AMOUNT OF $40,000

2) AUTHORIZING THE EXECUTION OF A GRANT AGREEMENT WITH SILICON VALLEY COMMUNITY FOUNDATION TO HELP PROVIDE COMPREHENSIVE DENTAL SERVICES TO LOW INCOME ADULTS; PROVIDE TWICE WEEKLY TRANSPORTATION BETWEEN SMMC AND UOP; AND DEVELOP AN ORGANIZATIONAL WORK PLAN FOR SUSTAINED FUNDING OF THE TRANSPORTATION PROGRAM FOR THE TERM JULY 1, 2007 THROUGH

JUNE 30, 2008, IN AN AMOUNT NOT TO EXCEED $40,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on January 4, 2005, the Board adopted Resolution No. 067134 accepting a grant in the amount of $40,000 from the Peninsula Community Foundation to fund transportation between San Mateo Medical Center (SMMC) and the University of the Pacific School of Dentistry (UOP); and

WHEREAS, on May 2, 2006, the Board adopted Resolution No. 067963 accepting a second year of support from Peninsula Community Foundation to continue funding transportation between SMMC and UOP; and

WHEREAS, Peninsula Community Foundation and Community Foundation Silicon Valley have merged to create the Silicon Valley Community Foundation; and

WHEREAS, the Silicon Valley Community Foundation has determined to continue its generous funding of the SMMC’s programs by making another grant of $40,000 to assist in providing comprehensive dental services to 700 low income adults through UOP and the SMMC’s Willow Clinic; provide twice weekly transportation between SMMC and UOP; and to develop an organizational work plan for sustained funding of the transportation program; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a grant award and an Agreement, reference to which is hereby made for further particulars, whereby the Silicon Valley Community Foundation will allocate funds for the provision of a program making comprehensive dental care available to low income patients of San Mateo County for the term July 1, 2007 to June 30, 2008, in the amount of $40,000; and

WHEREAS, this Board has considered the grant award and the Agreement and desires to accept the award and enter into the Agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to accept said award and execute said Agreement on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signatures thereto.
