AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND DENNIS ISRAELSKI, MD
THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and DENNIS ISRAELSKI, MD, hereinafter called "Contractor";
W I T N E S S E T H:
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
WHEREAS, the parties entered into an Agreement for infectious diseases services and direction of research on December 6, 2005; and
WHEREAS, the parties wish to amend the Agreement to increase the amount by $270,000 from $822,500 to $1,092,500 and extend the term by nine months for a new term of April 1, 2005 through March 31, 2008.
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
1.	Section 3.1 Term of the agreement is amended to read as follows: This Agreement shall commence on April 1, 2005, and shall continue for 36 months. Unless sooner terminated, this Agreement shall expire and be of no further force and effect as of the end of business on the 31st day of March, 2008.
2.	Original Schedule B is replaced in its entirety with Revised Exhibit B, (rev. December 18, 2007).
3.	All other terms and conditions of the agreement dated December 6, 2005, between the County and Contractor shall remain in full force and effect.
