BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE THE GRANT AGREEMENT FROM THE CALIFORNIA ENDOWMENT FOR THE TERM OF JANUARY 1, 2008 TO DECEMBER 31, 2009, FOR AN AMOUNT NOT TO EXCEED $345,750.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, to address the challenges facing uninsured adults living in San Mateo County, your Board formed the Blue Ribbon Task Force for Adult Health Care Expansion (Task Force) on June 26, 2006; and

 

WHEREAS, the Task Force’s recommendations created the San Mateo Access and Care for Everyone (ACE) program through a grant received from the State of California Department of Health Care Services.

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby The California Endowment shall provide funding to the San Mateo County Health Department, on behalf of the County of San Mateo, to support the evaluation of the ACE Program; and

 

WHEREAS, external evaluation consultants will be hired using the California Endowment funds to analyze improvements in clients’ health, as well as financial and delivery system efficiency gains over the evaluation period.

 

WHEREAS, the term of the Grant Agreement with The California Endowment is January 1, 2008 through December 31, 2009, for a total amount of $345,750;

 

WHEREAS, this Board of Supervisors has been presented with a form of the Agreement and has examined and approved the Agreement as to form and content and desires to enter into the Agreement:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo Health Department, and the Clerk of this Board shall attest the President’s signature thereto.
