BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH EDGEWOOD CENTER FOR CHILDREN AND FAMILIES FOR FSP SERVICES FOR CHILDREN AND TRANSITION-AGE YOUTH, EXTENDING THE TERM THROUGH JUNE 30, 2009, AND INCREASING THE AGREEMENT MAXIMUM BY $3,658,008 TO A NEW MAXIMUM OF $8,598,219

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the County Manager approved an agreement with Edgewood Center for Children and Families to perform start-up services for the implementation of FSP services for children, youth and transition-age youth for the term April 1, 2006 through June 30, 2006 for a maximum obligation of $100,000; and

 

WHEREAS, on June 20, 2006, your Board approved an Amendment to the agreement with Edgewood Center for Children and Families increasing the maximum obligation by $4,840,211 to a new maximum obligation of $4,940,211 and extending the term through June 30, 2008; and

 

WHEREAS, both parties now wish to enter into a Second Amendment to the Agreement to increase the maximum obligation by $3,658,008 to a new total obligation of $8,598,219, extend the term through June 30, 2009, and to add Therapeutic Behavioral Services for FY 2008-09; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Second Amendment to the Agreement, reference to which is hereby made for further particulars, whereby Edgewood Center for Children and Families shall provide full service partnership services for children, youth and transition-age youth, drop-in center services, flexible fund administration services, and Therapeutic Behavioral Services for FY 2008-09; and

 

WHEREAS, this Board has been presented with a form of the Second Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Second Amendment to the Agreement:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is authorized to execute said Second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
