BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

ADOPT A RESOLUTION AUTHORIZING ACCEPTANCE OF A GRANT AWARD FROM THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH (CDPH) TO PROVIDE PRIMARY HEALTH CARE, A FOOD PROGRAM, HOUSING AND SUBSTANCE ABUSE SERVICES TO INDIVIDUALS WITH HIV/AIDS, FOR THE PERIOD OF JULY 1, 2007 THROUGH JUNE 30, 2008, IN THE AMOUNT OF $719,930.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Grant Award from the California Department of Public Health, reference to which is hereby made for further particulars whereby San Mateo County Health Department Public Health and Environmental Protection/AIDS Program shall provide primary care, a food program, housing and substance abuse services to individuals with HIV/AIDS; and

WHEREAS, the term of the Agreement is July 1, 2007 through June 30, 2008, and the maximum dollar amount is $719,930; and

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
