RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT A01 TO AGREEMENT NO. 05-45217 WITH THE CALIFORNIA DEPARTMENT OF PUBLIC HEALTH TO PROVIDE 50 ADDITIONAL VIRAL LOAD TESTS FOR FISCAL YEAR 2007-08, INCREASING THE AMOUNT OF THE AGREEMENT BY $5,000 FOR A TOTAL OBLIGATION OF $155,000.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Amendment A01 to Agreement No. 05-45217, reference to which is hereby made for further particulars, whereby the California Department of Public Health will provide additional funding to the Public Health Lab for Viral Load Test Vouchers; and

WHEREAS, the term of the Agreement remains July 1, 2005 through June 30, 2008, and the maximum dollar amount is increased by $5,000 for a total obligation of $155,000; and

WHEREAS, this Board has been presented with this Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
