RESOLUTION AUTHORIZING THE EXECUTION OF A FOURTH AMENDMENT TO THE AGREEMENT WITH STANFORD UNIVERSITY TO CONDUCT A CLINICAL TRIAL ON CORONARY HEALTH PREVENTION, INCREASING THE AMOUNT PAYABLE TO THE COUNTY OF SAN MATEO BY $70,000 TO $669,129.50 AND EXTENDING THE TERM OF THE AGREEMENT TO MARCH 31, 2008

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on October 17, 2003, the Purchasing Agent approved an Agreement with Stanford University to conduct a clinical trial on Coronary Health Prevention; and

WHEREAS, the term of the original Agreement was September 1, 2003 to March 31, 2004 in the amount of $92,991 which Stanford University awarded to the County of San Mateo for the provision of clinical trials at four County clinics; and

WHEREAS, the Board has approved three subsequent Amendments to the Agreement which, collectively, extended the term of the Agreement to March 31, 2007 and increased the amount payable thereunder to the County to $599,129.50; and

WHEREAS, the parties wish to further amend the Agreement to increase the amount payable thereunder to the County by $70,000 to a total of $669,129.50 and to extend the term by one year to March 31, 2008; and

WHEREAS, the Board has reviewed and considered the Fourth Amendment and desires to enter into it.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be, and is hereby authorized and directed to execute said Fourth Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signatures thereto.
