RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AMENDMENT TWO TO THE AGREEMENT WITH THE AIDS COMMUNITY RESEARCH CONSORTIUM TO PROVIDE HEALTH EDUCATION AND A FOOD SERVICES PROGRAM, EXTENDING THE AGREEMENT TERM FROM

JUNE 30, 2008, TO JUNE 30, 2009, AND INCREASING THE CONTRACT AMOUNT BY $169,522, FOR A MAXIMUM OBLIGATION OF $491,300

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Amendment Two to an Agreement, reference to which is hereby made for further particulars, whereby the AIDS Community Research Consortium shall provide a health education, a food services program; and

WHEREAS, the term of the Agreement is extended from June 30, 2008, to June 30, 2009, and the maximum dollar amount has been increased by $169,522 for a maximum obligation of $491,300; and

WHEREAS, this Board has been presented with the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into the Amendment:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of the Board of Supervisors be and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
