RESOLUTION WAIVING THE REQUEST FOR PROPOSAL PROCESS AND AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH SIEMENS MEDICAL SOLUTIONS USA INC. FOR THE INVISION HEALTH INFORMATION SYSTEM AT THE SAN MATEO MEDICAL CENTER, AND EXTENDING THE AGREEMENT THROUGH OCTOBER 1, 2015, AND AMENDING THE NOT TO EXCEED AMOUNT TO $11,400,000; AND GRANTING SIGNATURE AUTHORITY TO THE COUNTY’S CHIEF INFORMATION OFFICER FOR MODIFICATIONS TO THE TERMS, SCOPE, AND CONDITIONS UP TO THE STANDARD CONTRACT AMOUNT.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 23, 2003, the Board of Supervisors by Resolution Number 066265 approved an Agreement in the amount of $12,000,000, for the term of September 23, 2003 to December 31, 2010, with Siemens Medical Solutions USA Inc., for Invision Health Information systems support and new applications; and

WHEREAS, this Board having considered the matter, has determined that it would be in the best interest of the County to approve an Amendment to the Agreement with Siemens Medical Solutions USA Inc. extending the term to October 1, 2015 and adjusting the Not to Exceed amount to $11,400,000; and

WHEREAS, this Board has determined that it is in the best interest of the County to adopt the Resolution and waive the Request for Proposal process.

WHEREAS, this Board has been presented with a form of the Amendment, has examined and approved it as to both form and content and desires to enter into same;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED

That the President of this Board of Supervisors be, and is here by authorized and directed to execute the Amendment as is approved by the County Counsel for and on behalf of the County of San Mateo the Clerk of this Board shall attest the President’s signature thereto.
That signature authority is granted to the Chief Information Officer for modifications to the terms, scope, and conditions up to the standard contract amount.
