RESOLUTION A) WAIVING THE REQUEST FOR PROPOSAL PROCESS, B) AUTHORIZING AN AGREEMENT WITH INFORMATIX INC. FOR DOCUMENT PREPARATION SERVICES IN SUPPORT OF IMAGING CASE FILES FOR THE DEPARTMENT OF CHILD SUPPORT SERVICES, AND C) AUTHORIZING THE DIRECTOR OF THE DEPARTMENT OF CHILD SUPPORT SERVICES OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Department of Child Support Services maintains 16,000 physical case files necessary for the establishment and enforcement of court ordered child support and desires to image the files in an effort to increase operational efficiencies, reduce costs and promote environmental conservation;

WHEREAS, the California Department of Child Support Services has developed and implemented a statewide imaging solution for all county child support offices;

WHEREAS, Informatix Inc. is uniquely qualified because of their local child support case management expertise, as a sole source vendor to provide physical case file preparation services necessary to leverage the existing statewide imaging solution;

WHEREAS, an Agreement has been presented to this Board of Supervisors for its consideration and acceptance, reference to which is hereby made for further particulars, whereby Informatix Inc. will provide physical case preparation services for imaging for the Department of Child Support Services for the period commencing June 1, 2008 through and including November 30, 2008 for a total obligation of $240,000 and;

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to waive the RFP process and execute said agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of the Department of Child Support Services or the Director’s designee shall be authorized to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
