RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE A SECOND AMENDMENT TO THE AGREEMENT WITH HORIZON SERVICES, INC., FOR THE CENTER FOR SUBSTANCE ABUSE TREATMENT INCREASING THE MAXIMUM OBLIGATION BY $69,051, FOR A NEW MAXIMUM OF $1,089,051, WITH NO CHANGE TO THE AGREEMENT TERM, SEPTEMBER 30, 2004 THROUGH SEPTEMBER 29, 2009.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a Second Amendment to an Agreement, reference to which is hereby made for further particulars, whereby Horizon Services, Inc. shall provide substance abuse treatment services at Maple Street and Safe Harbor Homeless Shelters; and

WHEREAS, this Second Amendment increases the obligation amount by $69,051 for the remainder of the term for a new maximum obligation of $1,089,051, with no change to the Agreement term of September 30, 2004 through September 29, 2009; and

WHEREAS, this Board has been presented with a form of the Second Amendment and has examined and approved it as to both form and content and desires to enter into this Amendment:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said Second Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
