RESOLUTION AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH HEALTHCARE COST SOLUTIONS TO PROVIDE AN INTERMIM MEDICAL RECORDS MANAGER, EXTENDING THE TERM OF THE AGREEMENT FROM AUGUST 20, 2007 TO NOVEMBER 30, 2007 TO AUGUST 20,2007 TO FEBRUARY 28, 2008 AND INCREASING THE AMOUNT BY $64,477.50 FROM $72,000 TO A MAXIMUM AMOUNT PAYABLE THEREUNDER OF $136,477.50

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on October 11, 2007, the County of San Mateo and Healthcare Cost Solutions entered into an Agreement pursuant to which Healthcare Cost Solutions provided the County with Medical Records Management Services; and

WHEREAS, , the parties wish to amend their Agreement to increase maximum amount payable thereunder by $64,477.50 from $72,000 to $136,477.50 and extend its term from August 20, 2007 to November 30, 2007 to August 20, 2007 to February 28, 2008; and;

WHEREAS, this Board has been presented with a form of the Amendment and has examined and approved it as to both form and content and desires to enter into it.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
