EXHIBIT A
PROGRAM DESCRIPTION
EDGEWOOD CENTER FOR CHILDREN AND FAMILIES
JULY 1, 2008 THROUGH JUNE 30, 2009
In consideration of the payments set forth in Exhibit “B”, Contractor shall provide the following services:
1. Needs Assessment and Implementation
Partner with San Mateo County Children and Family Services (CFS) to expand outreach efforts to families served by San Mateo County Self-Sufficiency Division.
Assess the needs of kinship families in San Mateo County by city through at least 12 CFS and Self-Sufficiency Unit presentations, annual Kinship caregiver Satisfaction Survey, twice a year community surveys to other youth and family service providers and community capacity building meetings with community-based organizations (CBOs). Edgewood will work with Children and Family Services (CFS) to prioritize the needs for Kinship families identified in the needs assessment. After the needs are prioritized through a collaborative process with CFS, Edgewood will work with CFS to implement solutions to the needs/issues identified.
2. Information and Referral
Provide information and referrals to San Mateo County Kinship/NREFM families. These families are either San Mateo County residents or San Mateo County CFS cases residing in other counties.
3. Community Capacity Building
Establish a presence by partnering with another CBO or independently in the cities of East Palo Alto and Daly City in addition to the San Carlos site.
Conduct at least three educational and skill building sessions [one per region] each year with staff from CBOs and HSA. These trainings will inform participants on issues for Kinship families and build and strengthen the skills of CBO and HSA staff in working with Kinship families.
4. Case Management
Provide case management services up to six months [case management is expected to always include information on KinGAP for Kinship families] to San Mateo County Kinship/NREFM families. Case management will be goal-oriented and strength-based. The case management’s empowerment approach will include an individual assessment, customized intervention and is outcome-driven. Case management can be a combination of home visiting and community-based interventions. Case management also includes participation on Team Decision Making (TDM) meetings [particularly at change of placements and case closures].
5. Support Groups
Conduct weekly bilingual [based upon the location’s predominant non-English language] support groups in East Palo Alto, Redwood City, San Carlos, S. San Francisco and Daly City. Support group facilitators shall come from the community where the support group occurs as much as possible.
6. Education
Conduct 30 parenting workshops for Kinship caretakers per year. Workshops will include topics identified in the needs assessment.
7. Counseling/Family Group Conferencing
Provide family conferencing services for KinGAP, Non-Needy Relative and Informal Kinship families. Family conferencing is a family-centered decision making model designed to empower families and unite clients and social service professionals in the common goal of child safety and well-being. The evidence-informed model of care planning also identifies and finds supportive adults who may be resources for a child and builds action plans based on consensual decisions to promote safety, stability and permanency in the lives of high-risk children. Family Conferencing is a voluntary process through which families develop plans to ensure long-term protection of the child.
8. Recreation
Provide bi-monthly recreation activities per year. Activities include sports events, picnics, cultural events, etc.
9. Transportation/Independent Living Program (ILP)
Provide and/or assist with transportation for Kinship youth attending the ILP.
10. Tutoring
Link Kinship families with tutoring services in their community.
EXHIBIT B
PAYMENT SCHEDULE
EDGEWOOD CENTER FOR CHILDREN AND FAMILIES
JULY 1, 2008 THROUGH JUNE 30, 2009
In consideration of the services the services provided by the Contractor pursuant to his Agreement and subject to the provisions of paragraph 3 of this Agreement herein, County pay Contractor in the manner described below, unless otherwise specifically authorized by the Children and Family Services Director or her designee:
1. County shall pay Contractor monthly for actual expenditures. Costs will not exceed FIVE HUNDRED SEVENTEEN THOUSAND ONE HUNDRED THIRTY SIX DOLLARS ($517,136). Contractor must provide detailed invoices on a format as specified by the County.
2. County may withhold all or part of Contractor’s total payment if the Director of Children and Family Services or her designee determines reasonably that Contractor has not satisfactorily performed the services described in Exhibit A.
3. In no event shall the total exceed FIVE HUNDRED SEVENTEEN THOUSAND ONE HUNDRED THIRTY SIX DOLLARS ($517,136) for the term of the Agreement.
4. Invoices shall be sent to: April Dunham, Human Services Manager, County of San Mateo, Human Services Agency, 400 Harbor Blvd., Bldg. B, Belmont, CA 94002. Payments shall be made within 30 days upon receipt of Contractor’s invoice.
5. All payments under this Agreement must directly support services specified in this Agreement.
6. Provision of services is subject to availability of State funds and acceptable program performance. In the event that the County does not receive the adequate funding from the State, the contract may be re-negotiated and/or rescinded.
7. County will give thirty (30) days prior written notice to Contractor of County's intent to withhold payment.
8. If County reasonably determines that circumstances warrant immediate action, County may withhold payment immediately, without the thirty (30) day waiting period, upon County's written notice with justification to Contractor.
EXHIBIT C
MONITORING PROCEDURES
EDGEWOOD CENTER FOR CHILDREN AND FAMILIES
I. Contractor agrees to the following outcomes:
1. Needs Assessment and Implementation
• Conduct needs assessment of kinship families in San Mateo County by city through:
o At least 12 CFS and Self-Sufficiency Unit presentations
o The annual Kinship caregiver Satisfaction Survey
o Bi-Annual (2 per year) community surveys to other youth and family service providers
2. Information and Referral
• Provide information and referral services to 200 Kinship/NREFM families per year.
3. Community Capacity Building
• Establish a presence in the cities of East Palo Alto and Daly City in addition to the San Carlos site.
• Conduct at least three educational and skill building sessions [one per region] each year with staff from CBOs and HSA.
o 80% of attendees will indicate an increased understanding of issues for Kinship families.
o 70% of attendees will indicate an increased comfort in working with Kinship families
o 50% of attendees will identify a next step to increase their agency/organization’s capacity to better serve Kinship families
4. Case Management
• Provide case management services [case management is expected to always include information on KinGAP for Kinship families] to 175 Kinship/NREFM unduplicated families per year.
o 80% of families receiving case management services will report increased self-advocacy skills as demonstrated by an improvement in measurement scores.
o 95% of children will remain in Kinship/NREFM placements or will be returned to their birth parent(s).
5. Support Groups
• Conduct weekly support groups in East Palo Alto, Redwood City, San Carlos, S. San Francisco and Daly City.
o A target of seven unduplicated individuals per support group per week will attend. 80% of individuals attending the support groups will report receiving appropriate support.
6. Education
• Conduct 30 parenting workshops for Kinship caretakers per year.
o 80% of families will report the information and effectiveness of the training was good or excellent.
o 70% of participants will report their intention to practice what they have learned.
7. Counseling/Family Group Conferencing
• Provide 360 hours of family conferencing services for KinGAP, Non-Needy Relative and Informal Kinship families.
o 80% will report increased functioning as a result of the family conferencing
o 80% will report increase advocacy ability as demonstrated by an improvement in measurement scores.
8. Recreation
• Edgewood will provide bi-monthly recreation activities per year
o 100 unduplicated individuals will participate in at least one recreational activity per year.
o 80% of families who participate will report the activities made a positive difference in their lives.
• Edgewood will provide 20 camperships for kinship kids.
9. Transportation/Independent Living Program (ILP)
• At least 15 kinship youth [aged 14+] receiving Kinship services will enroll in ILP
o Kinship youth will be on-time 85% of the time.
II. Contractor will be responsible for submitting quarterly activity reports, 6-month and year-end narrative reports and year-end financial reports. Reports will be submitted to: CFS Contract Monitor, Human Services Agency, 400 Harbor Blvd., Bldg. B, Belmont, CA 94002.
a. Quarterly Activity Reports are due on:
October 15, 2008; January 31, 2009; April 15, 2009; July 31, 2009
b. 6-Month report is due on: January 31, 2009
c. Year-end reports and year-end financial reports are due on: July 31, 2009
III. Contractor will submit to the CFS Contract Monitor at 400 Harbor Blvd. Bldg. B., Belmont, CA 94002, a roster of the Board of Directors and meeting dates with the year-end report.
IV. Contractor will submit to the CFS Contract Monitor at 400 Harbor Blvd. Bldg. B., Belmont, CA 94002, a financial audit, as soon as it becomes available.
V. County will conduct site visit during the term of the Agreement to review all aspects of program operations and review Contractor’s documentation related to case management. This site visit will be arranged in advance with Edgewood.

Child Abuse Prevention and Reporting
Contractor agrees to ensure that all known or suspected instances of child abuse or neglect are reported to a child protective agency. Contractor agrees to fully comply with the Child Abuse and Neglect Reporting Act, Cal Pen Code 11164 et seq. Contractor will ensure that all known or suspected instances of child abuse or neglect are reported to an agency (police department, sheriff’s department, county probation department if designated by the county to receive mandated reports, or the county welfare department) described in Penal Code Section 11165.9. This responsibility shall include:
A. A requirement that all employees, consultants, or agents performing services under this contract who are required by the Penal Code to report child abuse or neglect, sign a statement that he or she knows of the reporting requirement and will comply with it.
B. Establishing procedures to ensure reporting even when employees, consultants, or agents who are not required to report child abuse under the Penal Code gain knowledge of, or reasonably suspect that a child has been a victim of abuse or neglect.
C. Contractor agrees that its employees, subcontractors, assignees, volunteers, and any other persons who provide services under this contract and who will have supervisory or disciplinary power over a minor or any person under his or her care (Penal 11105.3) will be fingerprinted in order to determine whether they have a criminal history which would compromise the safety of children with whom Contractor's employees, subcontractors, assignees or volunteers have contact.
