AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND DANIEL J. BUCKLEY, M.D.
THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and DANIEL J. BUCKLEY, M.D., hereinafter called "Contractor";
W I T N E S S E T H:
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
WHEREAS, the parties entered into an Agreement for providing professional Ophthalmology services on September 12, 2006; and
WHEREAS, the parties wish to amend the Agreement to increase the maximum amount payable thereunder by $79,763.70, from $127,236.30 to $207,000.
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
3.	Original Exhibit B is replaced in its entirety by Revised Exhibit B, (rev. May 28, 2008), attached hereto and incorporated herein by reference..
4.	All other terms and conditions of the agreement dated September 12, 2006, between the County and Contractor shall remain in full force and effect.
