BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION A) AUTHORIZING EXECUTION OF AN AGREEMENT WITH YOUTH AND FAMILY ENRICHMENT SERVICES (YFES) TO PROVIDE DIFFERENTIAL RESPONSE CASE MANAGEMENT SERVICES FOR THE CENTRAL REGION FOR THE TERM JULY 1, 2008 TO JUNE 30, 2011, IN THE AMOUNT OF $947,571, THERE IS NO NET COUNTY COST; AND B) AUTHORIZING THE DIRECTOR OF THE HUMAN SERVICES AGENCY OR THE DIRECTOR’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, in February, 2008, a Request for Proposal (RFP) was conducted for the provision of Differential Response case management services in the North, Central and South regions of San Mateo County; and

 

WHEREAS, the County of San Mateo wishes to enter into an Agreement with YFES pursuant to which YFES will provide Differential Response services to at-risk families in the Central Region, for which San Mateo County’s maximum payment is $947,571 for the term of July 1, 2008 through June 30, 2011; and

 

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the same.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that:

A) The President of this Board of Supervisors is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto; and

B) Authorizing the Director of the Human Services Agency or the Director’s designee to execute contract amendments which modify the County’s maximum fiscal obligation by no more than $25,000 (in aggregate), and/or modify the contract term and/or services so long as the modified term or services is/are within the current or revised fiscal provisions.
