RESOLUTION AUTHORIZING (A) AN AMENDMENT TO THE AGREEMENT WITH COASTSIDE HOPE (COASTSIDE) TO PROVIDE FUNDING FOR THE REMAINING TERM OF THE AGREEMENT THE TERM REMAINS THE SAME, JULY 1, 2007 THROUGH JUNE 30, 2010, INCREASING THE AMOUNT BY $134,876 TO $215,350; $134,876 IS NET COUNTY COST; AND (B) THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED AN AGGREGATE OF $25,000, AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on May 24, 2007, the County Manager’s Office approved Agreement Number 71000-08-C006, waiving the Request for Proposal (RFP) process and authorizing Core Services in the amount of $80,474, for FY 2007-08 only; and

WHEREAS, the term of the Agreement will remain the same, July 1, 2007 through June 30, 2010, this Amendment will provide $134,876 for the remaining term of the Agreement for a new total obligation of $215,350; and

WHEREAS, this Board has been presented with the form of the Amendment to the Agreement and has examined and approved it as to both form and content and desires to enter into said Amendment.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that

A. The President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto; and

B. The Director of the Human Services Agency shall be authorized to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
