RESOLUTION AUTHORIZING 1) THE EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH PACIFIC RIM BUILDING SERVICES TO PROVIDE JANITORIAL SERVICES, FIVE DAYS PER WEEK, AT THE FAIR OAKS ADULT CLINIC, THE FAIR OAKS CHILDREN’S CLINIC, THE METHADONE CLINIC, THE SEQUOIA TEEN WELLNESS CENTER, THE WILLOW CLINIC AND THE SOUTH SAN FRANCISCO CLINIC EXTENDING THE TERM BY TWO YEARS FROM APRIL 15, 2007 TO APRIL 14, 2010 AND INCREASING THE MAXIMUM AMOUNT PAYABLE BY $158,287.50 FROM $96,722.50 TO $255,010 2) AUTHORIZING THE CHIEF EXECUTIVE OFFICER (CEO) OF SAN MATEO MEDICAL CENTER OR THE CEO’S DESIGNEE TO EXECUTE CONTRACT AMENDMENTS WHICH MODIFY THE COUNTY’S MAXIMUM FISCAL OBLIGATION BY NO MORE THAN $25,000 (IN AGGREGATE), AND/OR MODIFY THE CONTRACT TERM AND/OR SERVICES SO LONG AS THE MODIFIED TERM OR SERVICES IS/ARE WITHIN THE CURRENT OR REVISED FISCAL PROVISIONS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the San Mateo Medical Center (SMMC) requires janitorial services for its Fair Oaks Adult Clinic, Fair Oaks Children’s Clinic, the Methadone Clinic, the Sequoia Teen Wellness Center, the Willow Clinic, and the South San Francisco Clinic; and

 

WHEREAS, this Board has been presented with an Amendment to the Agreement with Pacific Rim Building Services for these services and has approved it as to both form and content and desires to enter into the same;

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute the Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Chief Executive Officer of San Mateo Medical Center or his designee shall be authorized to execute Amendments and minor modifications not to exceed $25,000 and to make minor changes in the type of services and activities provided under the Agreement.
